Hands On Physical Therapy would like to provide the best possible service to our patients. For faster service, please complete the patient forms before arriving to your first appointment. Also, please remember to bring your medical insurance card and your doctor’s prescription if required by your medical insurance provider.

On your first visit to Hands on Physical Therapy, a licensed physical therapist will evaluate your injury or condition, and discuss your goals and specific needs to help develop an individualized treatment plan. Your evaluation will take 45 minutes to an hour, and your proceeding visits will take about 45 minutes to an hour depending upon your injury or condition. It is very important to follow your individualized treatment plan in order to meet the goals that you set for yourself.
